Do I need to connect a fuse to protect the EPR input from a 12V battery of electric wheelchair or not?

Hi,
No, you do not need to connect anything to the EPR, there is already PTC fuse, Reverse Polarity Diode protection as also ESD protection. SO, Just use it as it is, and do not worry about polarity, if you connect it reverse, just will not work, nothing bad will happen.

There is a fuse box in your drivers side dash, just pull the cover straight out,
